I am using PowerGen 6.0.0.21 for PB9. I believe this to be the latest and greatest?
I did a full application bootstrap via PBG's. The bootstrap was successful with no errors. I then deleted a PBL and attempted to bootstrap that one pbl using the GUI interface. Nothing was added to the pbl. I also tried to sync the existing pbl (after deleting objects from the PBL and nothing was added back in.
I know that there are many pathing issues, etc., and the possibility it is failing silently, but I would think that after doing a successful full app bootstrap my PBG's, project oths, etc. should be fine.
Is there a known issue with this?

I tried the operations you are describing and didn't see a problem.
I had an application with 2 PBLs. I deleted one and then choose Library->Bootstrap Import->From PBG from the main menu. Then I tried using Syncrhonize with the same result.
Are all of your PBLs in the same directory? Are you getting any error messages at all?

Go into your project maintenance and change the PBL paths to relative. See if that makes a difference. After doing some more testing and tinkering, I find that if I hard code the PBL path, it works. If I use a relative pbl path, the bootstrap log correctly lists the path of the pbl, but nothing is bootstrapped from that pbl. If I bootstrap the entire app the relative pathing works, but not when bootstrapping a single pbl.
If you cannot reproduce this, I can send you my test app, which is only 2.6MB compressed in a zip file.
